Extended Care Program

The Extended Care Program is designed to enhance the care provided to our patients who have limited mobility residing in nursing homes or skilled facilities. Benefits include  

• increased communication between the patient, the facility, the Talbert provider, and family members
• increased visits from a Talbert provider
• daily or weekly visits by a Talbert provider to the facility

Who is eligible for the Extended Care Program?
Patients are eligible if they reside in a nursing home or a skilled nursing facility.

Who is involved in the care process?
A Talbert nurse practitioner, physician assistant, and doctor will provide the required care.

 Care Everywhere Program

Talbert Medical Group and Long Beach Memorial Medical Center, one of our preferred hospitals, now communicate through an electronic system called "Care Everywhere." This system enables us to securely transfer medical records, including lab test results, instantly between our two facilities.

Now our physicians have the information they need to treat patients at their fingertips and patients have the comfort of knowing that accurate medical records are used to make treatment decisions, whether they are at a hospital or in their physician's office.
